About Retatrutide

Built from 39 amino acids, retatrutide is a synthetic peptide designed to act on three metabolic receptors: GLP-1, GIP and glucagon. It contains non-natural amino acids and a C20 fatty-diacid side chain that lengthens its plasma half-life, the same approach used in semaglutide and tirzepatide. Few incretin-class compounds get more attention in current metabolic and obesity research.

Mechanism

It works on three metabolic receptors at the same time: GLP-1, GIP and glucagon. In the literature, GLP-1 and GIP signalling lowers appetite and slows gastric emptying, while glucagon signalling raises energy expenditure. How the three pathways combine is still being studied in clinical trials.

Evidence so far

The New England Journal of Medicine carried the Phase 2 results in 2023. Phase 3 trials are running, and the first Phase 3 results have been reported. The table below lists the main published figures. Retatrutide is not an approved medicine. Semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro) are.

The 2023 Phase 2 paper in the New England Journal of Medicine reported mainly gastrointestinal side effects: nausea, vomiting, diarrhoea and constipation. The investigators described them as dose-related and mostly mild to moderate.

Retatrutide is investigational. The EMA, the FDA and other regulators have not assessed or approved it, and its long-term safety is unknown.
